Printed forms can be emailed (to kcruz@holyspiritvb.org), dropped off or mailed. Please complete a form for each of your children. It is important that you do not delay your registration. The sooner we know who is joining us, the better we can prepare accordingly for group size, space and materials.

Most communication will be done via email this year so it is paramount that email addresses be clearly written and best phone contact information be given for your family.

Be assured that we will continue to monitor current events and adapt our programs accordingly. Your flexibility and understanding in advance, is greatly appreciated.

  • Is your or your child/teen’s temperature over 100.4 degrees?

  • In the past two (2) weeks, have you or your child/teen presented symptoms of COVID-19, such as a fever, chills, sore throat, cough, shortness of breath, muscle pain, nausea, vomiting, or diarrhea, with no alternative diagnosis?

  • In the past two (2) weeks, have you or your child/teen been exposed - less than six (6) feet for greater than 15 minutes - to anyone who has tested positive for or exhibits symptoms of COVID-19 (with no alternative diagnosis), without wearing the appropriate Personal Protective Equipment (PPE) as defined by the Centers for Disease Control and Prevention?

If you or your child/teen answered yes to any of the questions stated above, please do not attend the session.

At the church, leaders will make sure that:

  • Tables are 6 feet apart

  • Small groups will still practice social distancing when going to other areas in the building

  • Hand sanitizer stands are present throughout the building

  • If snacks are provided, they are distributed by designated leaders and are store-bought, individually-wrapped packages
